Each Volunteer is required to complete a Pennsylvania State Police Criminal History Records Check and a

  Pennsylvania Child Abuse History Clearance. 

  Each has a cost of $10 to be paid by the applicant. 

  The criminal history must be completed first.  A copy of your processed Criminal Record needs to be

  attached and returned with the Child Abuse History clearance request. 

  Pennsylvania Access To Criminal History (PATCH) website is the fastest and easiest way to complete

  and submit this form.

  1.  Click on Record Check

  2.  Click on New Record Check

  3.  Click on Accept

  Here you are going to enter your personal information:

  The lines indicated with an asterisk are required fields.

  Select Reason for Request:  DPW - Volunteer Work - Employment

  Complete all required fields and Click - Next. 

  Complete the Record Check Request form.  Enter any aliases or maiden name.

  Verify information as correct:  Spelling - SSN - Date of Birth

  Review all information - When satisfied that the information is correct, Click on Submit.

  Please complete the Credit Card Information and Submit.

  Check Status:  If it indicates No Record

  Click on the Control # (Rxxxxxxx)

  Click on Certification Form

  Click on Print - (suggestion - Print 2 copies)

  Check Status:  If it indicates Request Under Review

  Write the Control # for reference.  This does not indicate that you have a record - it does indicate that the

  PA State Police want to verify your name and SSN.  You will recieve in the mail a copy of your PSP,

  indicating No   Record or Record.  If you have a record - this will accompany the Certificate.

  Next, a Pennsylvannia Child Abuse History Clearance request must be completed.  Please request this form from

  the school office.  A copy of the above PSP criminal history certificate must be attached and sent along with this

  form and your $10 money order.
